At Barstow Community College (BCC), we take these concerns seriously and provide a formal process to help protect your identity and your academic record.

How to Complete the Process
Step 1 — Complete the Identity Theft Verification Affidavit
Fill out every section of the affidavit clearly and accurately. Be prepared to explain:

How you discovered the fraudulent activity

When you first became aware of the issue

Why you believe the activity was unauthorized
Incomplete information may delay processing.
Step 2 — Attach Required Documentation
You must attach copies of the following documents:

Valid government-issued photo ID (Driver’s License, State ID, or Passport)

Proof of residency at the time of the activity (lease agreement, utility bill, bank or insurance statement)

Copy of a police report filed with law enforcement documenting the identity theft
All documents must be clear, legible, and dated.
Step 3 — Submit Within 30 Business Days
Submit the completed affidavit and all required documentation within 30 business days of discovering or being notified of the identity theft.
