Student Services Forms

Student Conduct

Barstow Community College students are expected at all times to act in a manner consistent with BCC’s student academic integrity code and student behavior code of conduct. Both the student academic integrity code and behavior code of conduct are set forth in the student handbook. The student handbook is available to all students and is also available online.

This form is to submit an alleged behavioral violation by an individual of the Student Code of Conduct (i.e., alcohol, disorderly conduct, drugs, failure to comply, theft, vandalism, weapons, violent acts or threats of violence, etc.).

Academic Integrity

(Faculty Only)

In accordance with BCC Student Conduct Procedures (BP 5500), academic dishonesty cases that occur in the classroom shall be handled by faculty members first.

However, after faculty initial action has been taken, the faculty member shall complete this report to identify the student who is found responsible, the general nature of the offense, the action taken, and a recommendation as to whether or not additional administrative action is needed. The completed report is then sent by the faculty member to the Office of Student Life and Conduct.

PIVOT

(Promoting Inclusion, Voice, Opportunities, and Thriving)

Formerly known as BIT

Outreach and referrals to appropriate resources for students in need of assistance outside of the classroom and their coursework (i.e., absences due to medical reasons or extenuating circumstances, reports of mental health concerns, basic needs access, suicide ideation, and other issues of general concern).

If you have a concern please fill out the PIVOT Form.

Student Complaints

It is recommended that general concerns and complaints be filed at the lowest possible level, with the supervising manager of the area of concern.

Please note that this form is only the first step for us to receive more information about your case and is in no way meant to replace the personal help you will receive from our individual departments.

After reviewing this form, it will be shared with the appropriate BCC employee who can best address your concerns. Please provide as much detail about the incident as possible so that we can appropriately follow up.

If this is an emergency, or you need immediate assistance, please call 9-1-1

Grade Grievances

Students are encouraged to begin the academic grievance process informally by discussing the concern directly with the instructor and attempting to resolve the issue collaboratively. If the matter cannot be resolved through this informal communication, students may then proceed by submitting an official informal Academic Grievance Form to initiate a review with the Grievance Officer.

A student has the right to challenge a final grade if they believe the grade was assigned due to one of the following:

  • Mistake – An unintentional error made by the instructor.
  • Fraud – An intentional misrepresentation of facts that results in a negative outcome for the student.
  • Bad Faith – Any intentional act by the instructor that negatively affects the student’s grade.
  • Incompetence – Evidence that the instructor lacks the knowledge, skills, or abilities necessary to teach and fairly evaluate the course. Incompetence is usually pervasive and not restricted to one student or one incident.

Students who believe they have grounds for a grade grievance may submit the Informal Student Academic Grievance Form. After the form is received, the Grade Grievance Officer will contact the student to review the concern and answer any questions.
