EOPS/CARE Application

The EOPS Application is a crucial component and must be completed for consideration to the EOPS program. Aside from the application, there are other documents required to support a students’ eligibility for EOPS services.

Listed below are documents that a complete EOPS Application will contain:

• Complete EOPS Application
• Proof of BOGG A or B-
• This document can be obtained after a student applies for Financial Aid and is determined BOGG eligible.
• Proof of income:
• Agency Verification- Any agency verification can be used to prove a students receipt of CALWORKS/cash aid, General Assistance(GA), Social Security Income (SSI). The agency verification forms needs to be completed and stamped by the verifying agency. The Agency Verification form is available online or in the EOPS office.

OR

• Statement of Fact- A statement of fact form is required if an applicant didn’t file for the prior year income tax. This statement provides sworn proof of a student having earned no taxable income for the previous year. The Statement of Fact form is available online or in the EOPS office.

OR

• Income Tax Form
• A copy of the previous year tax return that documents the number of people in your family and the total family income.

• Transcripts from any previous colleges (If applicable)
• Copy of Assessment Test results
• Copy of current semester schedule
